How the Plan Works
You must use a VSP network provider in order to receive full benefits. For a list of participating providers:
  • Call VSP at 1-800-VSP-7195, or
  • Visit www.vsp.com.
    • Click "Find a Doctor" at the top of the homepage.
    • Select that you're not a VSP member, and enter your geographic information. (Although you may already be a VSP member, this is the simplest way to find participating providers in your area.)
VSP includes 100% coverage for annual eye exams and eyeglass lenses, as well as generous coverage for frames and contact lenses. To receive these benefits, a VSP network provider must perform vision care services.
VSP also provides out-of-network benefits. You'll receive a lesser benefit and typically pay more out-of-pocket. See the Summary Plan Descriptions for more details.
An Example of Vision Plan Savings
VSP Member Promise
If you're not satisfied with a visit to a VSP network provider, you can visit a second VSP provider free of charge. This second visit doesn't count towards the plan's limitations. Contact VSP at 1 800 VSP 7195 for more information.
Assume that you are single and you will need an eye exam and new eyeglasses for 2007.
