Plan pays 50% and no lifetime maximum (limited to one full treatment plan per eligible member)
Plan pays 50%, with no deductible (lifetime maximum of $2,000 per eligible member)
* Both in-network and out-of-network benefits are available. In-network benefits are based on negotiated fees. Out-of-network benefits are based on reasonable and customary charges.
