Appealing Benefit Claims That Are Denied
If your claim for benefits is denied, you are entitled to appeal the denial. To file an appeal, you (or an appointed representative) must do so in writing within 180 days after receiving the claim denial notice, as described in the benefits Summary Plan Descriptions.
Appeals should be filed as described in the Summary Plan Descriptions and submitted to the appropriate address below.
